Durante il sec. XII, due terremoti compromisero gravemente la Chiesa "mosaicata" e si decise quindi la costruzione di un nuovo tempio.
I lavori dell'attuale costruzione, attribuita ad Alfano da Termoli, iniziarono verso la fine del secolo, finanziati da ricchi commercianti di Ravello.
La Cattedrale è suddivisa in tre navate da pilastri cruciformi e presenta una copertura a capriate nella navata centrale e volte a crociera in quelle laterali.
Nel corso dei secoli la Cattedrale subì calamità naturali e saccheggi che la devastarono notevolmente.
A metà del XVIII secolo l'interno subì una trasformazione barocca da cui venne liberata negli anni trenta, quando vennero alla luce i mosaici pavimentali e i resti delle absidi dell'edificio religioso preesistente.
Con i recenti interventi di restauro sono venute alla luce altre parti del mosaico pavimentale e un'ampia area cimiteriale sotto i locali della sagrestia, risalente al IX secolo negli strati più antichi. Il 31 dicembre 1761 nella cripta della Cattedrale furono rinvenute le ossa di S. Basso, patrono di Termoli; mentre, nel maggio del 1945 vi furono rinvenute quelle di S. Timoteo, discepolo di S. Paolo.
The Cathedral is built on the highest point of the Borgo Vecchio, in the place of the oldest urban settlement, as evidenced by some archaeological finds dating back to the Bronze Age.
The first building stands, probably on the ruins of an ancient pagan building of which there are no traces. Over the centuries the Cathedral suffered natural calamities and looting that devastated it considerably.
Towards the middle of the 18th century, floor mosaics and the remains of apses of the pre-existing religious building came to light.
With the recent restoration interventions, other parts of the floor mosaic came to light and a large cemetery area under the premises of the sacristy, dating back to the 9th century in the oldest layers. On 31 December 1761 the bones of S. Basso, patron saint of Termoli, were found in the crypt of the Cathedral; while, in May 1945, those of St. Timoteo, the disciple of St. Paul, were found.
